Hilco in TX is offering a box of 5 of them but you take the form into one of their offices not Walmart. The form was available in their magazine (they send it free to all their customers) Texas Co-op or something like that and is also available when they are at events like expos. If you are a TX Hilco member and didn't get a form, you should contact them and see if you can get a form at the office.

In order to receive this you must be a customer of Duke Energy...correct? Or is it for all residents in Ohio?
